A form for new participants in an Afghan Mujahid camp to sign out before entrance. The form begins by setting down a set of rules for students to obey. If they do not follow the rules, they may be released from the camp. At the end of a form is a pledge to abide by the above stated rules. The next section of the document is an information form for all new recruits, including information such as name, place and date of birth, education, profession, religious credentials, security status, manner of transportation, military history, health and marital status, political affiliation. The form ends with a pledge that all above information is correct.
