This document is an al-Qa`ida in Iraq pledge for traveling abroad to seek medical treatment, signed by an individual named of Mustafa Qudrah “Adawi using the alias Abu Mus`ab. By signing the pledge for traveling abroad for medical treatment Abu Mus`ab agreed to certain requirements. He completed the pledge by providing his name, telephone number, alias, signature, and thumbprint.
