This document is a blank pledge for ”brothers” who wish to travel abroad for medical treatment. The document stipulates the required information necessary for filling out the pledge, as well as the terms of the agreement.
The pledge for traveling abroad for medical treatment requires authorization from the Emir of the region, a completed application form, attendance at a security briefing, and a signature. By entering into the contract to travel overseas for medical treatment the fighter agrees to follow all orders given by the Emir of the region, to refrain from disclosing any information and not to return to Iraq without orders to do so.
