This document is an administrational template that carries detailed personal information for foreign fighters who wish to join the Islamic State of Iraq. It is headed with the title “Personal Information of the Immigrants,” and stamped with the stamp of “The Islamic State of Iraq.” It carries several questions that needed to be answered by foreign fighters, however, in this document, not all the questions were answered. This one, in particular, for foreign fighter“Badr Shori” in which he provided the following information: his real name, alias, country of origin, phone number, DOB, the amount of donations he made for the ISI, the coordinator [who facilitated his trip], route he took to reach Syria, names of people he met in Syria, the amount of money he brought with him, the amount of money he was charged in Syria, what will be his work in Iraq (martyr) and what was his occupation in his country.
